INSTRUCTIONS AND HELPFUL INFORMATION

PREPARATION BEFORE YOU LEAVE HOME:

1. Mark all luggage and other personal items with your name and home address.

2. A pen and paper will be required every day. Please be sure to pack these items.

3. Bring any special medication you require.

4. Check the "dress" section below and pack appropriate clothes including several changes.

5. Pack toilet articles such as a comb, a hair dryer, toothbrush/paste, shampoo, lotions, etc.; whatever you need to be presentable each day.

TRAVEL TIPS:

1. Anyone less than 18 years of age should travel with medical authorization in case of emergencies. You should carry a copy of the Youth Health Card in this packet in case of an emergency.

2. Everyone should carry personal identification at all times including the name and telephone number of a contact person in case of an emergency. On your first travel day (Saturday, May 1), call OUR CONTACT'S NAME at (xxx)xxx-xxxx if you have an emergency or a change of travel plans. If there is no answer, please leave a message as we will be checking voice mail frequently.

DRESS CODE:

Dress for the conference will generally casual. This means you can wear jeans and t-shirts/sweatshirts during most days, for evening meals and time on your own. Please bring at least one special outfit for the Youth Roundtable and other high profile activities. Overall, you should have a "cleaned-up" look and clothes must be neat in appearance.

CODE OF CONDUCT

IMPORTANT! The following are the rules and conditions for members of the National Town Meeting Youth Roundtable and INET News-team. Please read these carefully and have your parents look at them with you. You must sign at the bottom and also have your parent or guardian sign to demonstrate that you understand the rules. In order to participate in the program, you must return a signed copy of this form before the start of the meeting to OUR CONTACT HERE. Those who violate the conditions of this contract will be asked to leave immediately, at the travel expense of your parents.

The Youth Voices Program is alcohol-free. Possession or use of alcoholic beverages or illegal drugs is prohibited.

Smoking or using other tobacco products is prohibited.

Guests must respect personal and public property. Repair costs for damages incurred to property will be billed to the group responsible for such damages.

Quiet is to be observed in sleeping room areas from 11:30 p.m. to 7:00 a.m.. You are to be in your own room from 11:30 p.m. to 7:00 a.m.. Curfew time is 11:30 p.m. and you are expected to be in your room and quiet by this time.

You are expected to participate in all scheduled event activity periods during your stay.

I have read and understand the above code of conduct. If I break this agreement or my conduct is not satisfactory to the chaperons, I understand that I will be sent home early at the expense of my parents.

I am of the opinion that can SAFELY PARTICIPATE in the Youth Roundtable and INET News-team and that he or she has no contagious or communicable disease. His /her health is POOR, FAIR, GOOD (circle the term which applies) and he or she has had no illness within 30 days prior to departure. In case of an emergency while participating in the meeting, permission is given to physicians to perform needed treatment. I will assume all financial obligations incurred if not covered by insurance.
