Page one of the Narrative Responses for the Mustard Seed Foundation proposal is a Personal Faith Statement from the person administering or receiving the grant. This should be Bishop Hathaway. Please follow the guidelines which have been established by the Mustard Seed Foundation as described below.

PERSONAL FAITH STATEMENT:

1. Describe what you believe to be the essentials of you Christian faith. Include several sentences with "I believe..." Personal testimony and theological understandings both are appropriate seconday understandings.

2. Define your commitment to faith in Jesus Christ as Savior, evidence of His Lordship, Biblical emphasis, and your thoughts on the role of primacy of the local church. Do not submit a signed copy of the organization's faith statement in place of this more personal expression of your beliefs.

NOTE: Please limit this to one full type written page. Write the statement in Microsoft Word and paste the text in the dialog box. You may also attach a file to the report.
